台灣中老年健康因子及健康老化長期研究

Healthy Aging Longitudinal Study in Taiwan(HALST)

在2019年男性的平均壽命達到77.7歲,女性達到84.2歲,由於壽命的延展,台灣的人口逐漸老化,目前65歲以上的人佔全人口的14.1%。這個比率將逐漸增加,在2026年時估計將達到全人口的21.2%。人口老化相繼而來的通常是慢性病及老人症候群的盛行率增加,進而提高國家醫療成本。

本計畫是一個多中心長期追蹤的研究計畫,本研究重視人口老化的公共衛生議題,因此,自2009 年起,於北、中、南、東,共七個地區,針對55 歲以上中老年人進行長期追蹤調查。除了每五至六年一次的家庭訪視與健康檢查外,期間每一年也會利用電話訪視追蹤個案的健康情形與變化。由於本計畫原追蹤世代的年齡層隨著追蹤年數增加而增加,以及個案數因為遷徙、疾病或死亡等因素而流失或減少,為維持本計畫之代表性,故於2023年起建立新的追蹤世代

研究內容包含個人人口學基本變項生活型態飲食運動習慣認知功能精神狀態評估身體活動功能慢性病狀況生化檢查生物標記,以及基因和環境相關因子等,以了解中老年健康老化之生理、心理與社會功能的狀態與相互影響情形。因此規劃延緩老化失能的策略,提供政策參考並促進銀髮族健康。

研究目的

本計畫將可提供政策制定者將資源分配於促進健康生活型態上,以使得老年人能延緩罹病和失能開始的時間,維持高品質的生活並降低醫療成本。

執行重點:
  • 影響中老年人身體功能、衰弱症(含相關疾病狀態)及其變化之因子之研究
  • 健康的生活型態對中老年人心血管及代謝疾病與癌症的預防,以及生活品質與身體活動功能所帶來的益處之研究
  • 中老年人心血管及代謝症候群個人與環境層次之危險因子之研究
  • 中老年人精神疾患之相關因子及其對健康影響之研究
  • 中老年人衰老/成功老化及相關慢性病的生物、遺傳標記與環境因子之研究
  • 建置高齡低劑量斷層掃描(GOLD-CT,Geriatric Observation LDCT)
特點:
  • 參與者具全國城鄉之差異性,長期追蹤超過12年,串聯衛生福利資料科學中心(含健保資料檔、死亡檔、癌登癌篩檔等),各項與健康相關的結果完整、可信度高

研究方法

原追蹤世代:
  • 針對第一期所收錄的55 歲以上中老年人進行長期追蹤調查,每五至六年一次的家庭訪視與健康檢查,各期之間進行三次電話追蹤訪視(每一至二年一次),並串聯衛生福利資料科學中心,追蹤期間發生的醫療服務使用、新發之健康相關事件、死亡等資訊。
新追蹤世代:
  • 依照原追蹤世代的標準作業流程執行,進行長期追蹤調查。

    第一期在全台北中南東七家醫院附近社區收錄55歲以上中老年人,共5,663位;第二、三期則持續對第一期收錄之中老年人進行長期追蹤與訪視。

    由於本計畫原追蹤世代的年齡層隨著追蹤年數增加而增加,以及個案數因為遷徙、疾病或死亡等因素而流失或減少,為維持本計畫之代表性,故於2023年起建立新的追蹤世代。

資料統計截至 2026/08/09,並持續進行收案及長期追蹤。

世代建立及追蹤

2008年

2008年
  • 此計畫於2008年1月1日開始執行,前9個月由本計畫之計畫主持人、協同研究人員及國外顧問教授規劃計畫的推行、操作手冊之制定,人員訓練及預試。基線檢驗從2008年10月開始預試並持續進行,資料之品質做持續性的查核,定期舉行會議,討論有關檢驗、資料品質、學術研究方向、資料分析及論文準備撰寫等議題。

2009年

2009年
  • 針對北中南東共七家醫院(城市、鄉村約各半)附近社區之55 歲以上中老年人共5663人(第一期),進行訪問資料、身體檢查、血液及尿液樣本採集,以進行生物及遺傳標記研究。各期之間持續進行三次電話追蹤訪視(每一至二年一次),並串聯衛生福利資料科學中心,追蹤期間發生的醫療服務使用、新發之健康相關事件、死亡等資訊。

2013年

2013年
  • 第二期共4232人,針對第一期所收錄的中老年人,重新做完整的一次家庭訪視、臨床檢查,以及三次電話追蹤訪視。我們計畫針對與心血管疾病,發炎及老化以及精神健康狀態相關之生物及遺傳標記所代表之臨床意義及後續應用做進一步的研究。

2020年

2020年
  • 目前進行第三期追蹤研究,針對先前已參加第一期及第二期計畫所收錄的中老年人進行追蹤訪問及檢驗,新增低劑量電腦斷層檢測(LDCT),並與現有HALST世代結合,成為臨床、檢驗、影像等高齡資料庫。在各子計畫做更深入的探討,特別有關疾病狀態的變化,擬建立有效率的風險預測模式。

2023年

2023年
  • 由於本計畫原追蹤世代的年齡層隨著追蹤年數增加而增加,以及個案數因為遷徙、疾病或死亡等因素而流失或減少,為維持本計畫之代表性,故於2023年起建立新的追蹤世代。新世代依照原追蹤世代的標準作業流程執行,進行有關健康老化之長期追蹤研究。
